Monitoring report
A monitoring report is the document a project submits for each verification, setting out what was measured during the period, how, by whom, and what it amounts to. It is what the verifier checks and what the registry issues credits against.
It follows the structure the methodology requires: the period covered, the data collected, the calculations, the deductions applied, and any departure from the plan with the reason.
A good one lets a reader trace each reported figure back to the raw record it came from. That traceability is what the verifier spends most of the visit on.
